3、投标人资格要求

投标人应具备的资格或业绩：1) 投标人具有有效期内的营业执照（境内投标人）/企业经营许可证明文件（境外投标人），需提供相应证明复印件； 2) 投标人必须是制造商，本项目不接受代理商投标； 3) 投标人须持有有效期内的国家市场监督管理总局（或总局授权省级市场监管部门或省级市场监管部门）颁发的有效期内的压力容器制造许可证（D级或以上）。如旧证已完成换新，提供相应等级的特种设备生产许可证，需提供证书相应复印件； 4) 投标人自2012年1月1日至投标文件递交截止时间前7日（时间以验收报告时间为准），应在EO/EG装置中至少具有1套氧气混合器的运行业绩。请提供业绩表，并出具销售合同复印件及验收报告，合同须能体现签订时间、装置名称、货物名称、数量、签字盖章页；报告须能体现设备名称、设备验收合格时间、双方签字或盖章。若投标人投标文件提供的业绩表及所出具的证明材料不满足上述全部要求，则视为没有业绩； 5) 投标人应当提供投标人开户银行在开标日前三个月内开具的资信证明原件或复印件； 6) 本项目不接受联合体投标。

3.Qualification Requirements For Bidder

Qualifications or Performance：1) The bidder has a valid business license (domestic bidder)/operation li-cense (foreign bidder), and a copy of the corresponding certificate is required; 2) The bidder must be a manufacturer, the equipment agent is unaccepted for this bidding; 3) The bidder shall hold the pressure vessel manufacturing license (Grade D or above) issued by the State Administration for Market Regulation (or the provincial market regulatory authority authorized by the State Administration for Market Regulation or the provincial market regulatory authority) within the valid period. If the old certificate has been replaced, the corresponding level of special equipment production license shall be provided, and the corresponding copy of the certificate shall be provided; 4) From January 1, 2012 to 7 days before the deadline for submission of bidding documents (the time is subject to the final acceptance certificate materials), among the EO/EG plants, the bidder shall have at least 1 operational performance of O2 Mixer. Please provide a performance table, final acceptance certificate materials, and issue a copy of the sales contract and final acceptance certificate materials, sales contract must reflect time of contract signing, the name of the plant, name of goods, quantity, and signing/stamping page, final acceptance certificate materials must reflect the name of plant, the time of final acceptance certificate, and signing/stamping page. If the performance table provided in the bidding documents and the materials issued by the bidder do not meet all the above requirements, it will be deemed to have no performance; 5) The Bidder shall provide the bank reference letter issued within three 3 months prior to the time of bid opening in original or its copy; 6) The consortium will not be accepted.
